As a parent or guardian of a student at Graystone Academy Charter School, I have read the above information about the appropriate use of computers at the school, further I have explained it ot my child and understand this agreement will be kept on file at the school.

I release the Graystone Academy Charter School and its personnel from any and all claims and damages of any nature arising from my child’s use of, or inability to use, the school network or the Internet, including but not limited to those claims that my arise from the unauthorized use of the system to purchase products or services.

I understand that from time-to-time the school may wish to publish examples of student projects, photographs of students, and other work on an Internet accessible world wide web server.

I give permission for _________________________ to access, produce, and communicate information on the Internet. I understand and agree that the misuse of this privilege by my child will terminate their access to the network and the Internet.
